Welcome, and thank you for caring about cats and kittens in need! Whether you are looking to adopt, provide temporary boarding for your own cat, request help with a litter of kittens, or have a cat microchipped, we’re grateful for your interest in working with Kitty’s Kitten Rescue.

This page contains all of the forms needed to request services through our rescue. Please read the information below and select the form that best fits your needs.

Available Forms

Adoption Application

Use this form if you are interested in adopting a cat or kitten and providing a permanent, loving home.

Foster Application

Use this form if you would like to temporarily foster cats or kittens in rescue care until they find permanent homes.

Temporary Boarding Request

Use this form if you need short-term boarding for your own cat or kitten due to circumstances such as:

• • Spay/neuter recovery
  • Medical treatment or recovery
  • Hospitalization
  • Moving
  • Housing emergencies
  • A mother cat preparing to give birth
  • Other temporary situations

Please note that temporary boarding is subject to space availability and approval.

Microchipping Request Form

Use this form to request microchipping services for your cat or kitten. A microchip provides permanent identification and greatly increases the chances of being reunited with your pet if they become lost.

Kitten Intake Request Form

Use this form if you have found kittens and are requesting assistance from Kitty’s Kitten Rescue.

Examples include:

• • Orphaned kittens
  • Abandoned litters
  • Stray kittens
  • Mother cats with kittens
  • Kittens needing rescue placement

Submitting an intake request does not guarantee acceptance. Intake decisions are based on available space, foster homes, medical needs, and rescue resources.

How to Apply

Step 2: Complete the Form

Please answer all questions completely and honestly. Providing accurate information helps us process your request quickly and determine the best outcome for each cat or kitten.

Step 3: Submit Your Form

After completing the form, click the Submit button. You should receive confirmation that your submission was received.

After submitting your form, please send us a message through our official Facebook page.

This important step helps ensure your submission is seen promptly and allows us to respond more quickly.

A simple message such as:

“Hi! I just submitted a [Form Name] for [Your Name].”

is all that’s needed.

What Happens Next?

Once we receive your form and Facebook message, we will review your submission.

Depending on the type of request, we may:

  • Review veterinary records
  • Contact references
  • Request additional information
  • Schedule an appointment or meet-and-greet
  • Place your request on a waiting list if space is unavailable

Please remember that submission of any form does not guarantee approval, acceptance, intake, boarding placement, fostering, adoption, or rescue services.

We will contact you by phone, text message, email, or Facebook Messenger regarding the status of your request and any next steps.
